Perennial Continuum is designed to ask very little of you. Most members spend less than two hours with us before they never need to think about it again.
Baseline cognitive mapping
You meet your Continuity Consultant for an unhurried conversation about your life, your family and what matters most to you. This baseline helps us understand who you are, so that nothing essential is lost later. There are no needles, no machines and nothing to prepare.
If Continuum is right for you, your application is reviewed and you receive your acceptance by post, along with your member card.
Peace of mind
Go home and live. Keep your member card in your wallet and tell the people closest to you about your plans. The card lets hospitals, care homes and first responders reach us the moment they need to.
Continuity
When the time comes, our Continuity Response team comes to you, wherever you are, and your presence is carried forward with the care it deserves. Your family doesn't need to arrange a thing.
From then on, you remain with us: contributing, connected and never alone.

Minutes matter. We're already on our way.
Our Continuity Response team works around the clock across Metro Vancouver, so that every member receives the same prompt, dignified care.

Who can join?+
Continuum is available by application. Each year only a small number of people are accepted, based on their baseline mapping and the contribution they are able to make.
Does it hurt?+
No. Baseline mapping is a conversation. Everything that follows is quick, dignified and entirely painless.
What does it cost?+
Your consultant will explain your options in person. For many members, enrolment is fully sponsored.
Can I change my mind?+
Continuum is designed to be permanent. That is what makes it meaningful.
What will I be doing?+
Members remain present with their families and continue to contribute to meaningful Perennial initiatives. Most say they have never felt more useful.
Where is my information kept?+
Securely, and permanently.
